Vaccine Tracker Form: https://forms.gle/QMhZ1QbJLwksNTH26 This private Google Form will ask for proof of full vaccination with your vaccine card if you’d like to attend in-person events. Only board is allowed to see this form, and it will not be distributed. We will deny in-person events to those who deny proof of vaccination = keeps the club/people we contact safe and follows safety law (CSULB require vaccinations for all students regardless of online/in-person status unless there are important reasons). Online alternatives are already made and will be enforced regardless of vaccinations so we’re trying to accommodate for everyone. Most CSULB CKI events will be online because In-person events are limited in capacity and frequency until other further school instruction.
